US probe agency faults Boeing, FAA in 737 MAX door plug blowout The U.S. National Transportation Safety Board (NTS said Tuesday multiple systemic failures led to a mid-air door panel blowout of a Boeing jet last year, faulting Boeing and the Federal Aviation Administration (FAA) for training and oversight failures, reported Xinhua.

Boeing failed to "provide adequate training, guidance and oversight" to its factory workers, the investigative agency reported in a news release.

In an investigation spanning some 17 months, the NTSB found that four bolts securing the door panel were not in place in a new Boeing 737 MAX 9 jet during its production at Boeing's factory.

The door panel flew off the aircraft of Alaska Airlines in mid-air shortly after the jet took off on Jan. 5 last year from Portland, the U.S. state of Oregon. The crew made an emergency landing that ensured the safety of all people on board.

Boeing's manufacturing and safety oversight, combined with the FAA's ineffective inspections and audits, led to the malfunction, the NTSB noted.

Both Boeing and the FAA have improved training and inspections since the mid-air emergency occurred, but they need to better identify such risks to prevent them from sneaking through again, the NTSB said.

Iran extends flight suspension, Mossad chief says to continue operations Iran has extended the suspension of all domestic and international flights until Thursday afternoon, the Ministry of Roads and Urban Development said on Wednesday, citing passenger safety concerns following a recent conflict with Israel, reported Xinhua.

The decision was made "in light of recent developments and to ensure the safety and security of passengers and flights," ministry spokesperson Majid Akhavan said in a statement published on the ministry's website.

Iran closed its airspace on June 13 after Israeli airstrikes targeted Tehran and other locations. The extension of the closure comes despite a U.S.-brokered ceasefire that took effect early Tuesday.

Separately, Alaeddin Rafi'zadeh, head of Iran's Public Administration and Recruitment Organization, said government offices and state-run institutions would resume normal

Meanwhile, David Barnea, chief of Israel's Mossad intelligence agency, said the organization will continue its activities in Iran, according to a video released by Mossad on Wednesday, said Xinhua.

The video shows Barnea addressing operatives at the agency's operational headquarters on Tuesday, just hours after a ceasefire between Israel and Iran took effect.

"We will continue to keep a close eye on all of Iran's projects that we already know in the most in-depth way. We will be there as we have been there until now," he said.

He noted that, "With accurate intelligence, advanced technologies, and unimaginable operational capabilities, we helped the Air Force disrupt the Iranian nuclear program, establish air superiority over Iran, and reduce the missile threat, thereby ensuring the security of the citizens of Israel."

In the video, Barnea also thanked the U.S. Central Intelligence Agency "for the joint activity."

On June 13, Israel launched major airstrikes on different areas in Iran, including nuclear and military sites, killing several senior commanders, nuclear scientists and many civilians. Iran responded by launching several waves of missile and drone attacks on Israel, inflicting casualties and heavy damages.

The ceasefire between the two countries was declared on Tuesday.

Iran ratifies plan to halt cooperation with IAEA The Iranian parliament on Wednesday approved the general outlines and details of a plan to suspend the country's cooperation with the International Atomic Energy Agency (IAEA), reported Xinhua.

The bill was approved at an open session of the parliament in the Iranian capital Tehran, with 221 votes in favor and one abstention, reported the ICANA news agency affiliated with Iran's legislative body.

According to the report, the plan requires the Iranian administration to suspend its cooperation with the IAEA, the United Nations nuclear watchdog.

The plan stipulates that, given the violation of Iran's national sovereignty and territorial integrity by Israel and the United States through attacking the country's peaceful nuclear facilities and jeopardizing its interests, the Iranian government is duty-bound to immediately suspend any kind of cooperation with the IAEA based on the Non-Proliferation Treaty (NPT) and the Comprehensive Safeguards Agreement until a number of preconditions are met.

It lists the conditions as ensuring respect for Iran's national sovereignty and territorial integrity, the security of the country's nuclear centers and scientists within the framework of the UN Charter and the complete recognition of Iran's inherent rights under the NPT, especially the enrichment of uranium on Iranian soil.

Elaborating on the plan, Seyed Mohammad Nabavian, an Iranian lawmaker, said it was not focused on Iran's withdrawal from the NPT, as the country was committed to the treaty and had announced that its nuclear activities were all peaceful, ICANA reported.

Following Iran's attack, U.S. President Donald Trump announced that a ceasefire between Iran and Israel would begin around 0400 GMT Tuesday. Both sides later confirmed the start of the ceasefire.

Deaths from Russian strikes in central Ukraine rise to 20 Tuesday's Russian ballistic missile strikes in Ukraine's central Dnipropetrovsk region have killed 20 people and injured about 300 others, regional governor Sergii Lysak said Wednesday, reported Xinhua.

Eighteen were killed in the city of Dnipro, and two others died in the nearby city of Samar when an infrastructure facility was hit, Lysak said.

In Dnipro, the raids caused wide destruction, damaging almost 50 high buildings, about 40 educational institutions, 8 healthcare locations and several infrastructure facilities, according to Mayor Borys Filatov.

"In terms of damage, this is probably one of the most audacious attacks on Dnipro," Filatov said on Telegram.

10 killed in gunfight at patron saint festival in Mexico At least 10 people, including children, were killed in a gunfight Tuesday night at a patron saint festival in Irapuato, a city in central Mexico's Guanajuato state, President Claudia Sheinbaum confirmed Wednesday morning, reported Xinhua.

"It was actually a clash but unfortunately children died," the president said at her daily press conference at the National Palace in Mexico City.

"What happened is very regrettable," Sheinbaum said, adding the investigation into the incident is underway and pledging to work with Guanajuato Governor Libia Dennise Garcia Munoz Ledo to address the ongoing violence in the state.

"We will continue to work together with the governor to the extent possible," the president said.

Irapuato's local government issued a statement condemning the attack, which occurred at a private home and left several people with gunshot wounds.

The local government acknowledged Irapuato has become a hotspot of violence by organized crime and reiterated its commitment to restoring peace.

"I express my solidarity and condolences to the affected families, and reiterate our commitment to the safety of the people of Guanajuato," the statement from the city said.
