In an era of preventative medicine, erectile dysfunction (ED) may be one of the most underused biomarkers of early cardiovascular disease (CVD). Growing evidence shows that ED is often an initial manifestation of endothelial damage and inflammation which share the same pathophysiology underlying the development of atherosclerosis leading to CVD.1 ED often precedes the first myocardial infarction by half a decade, offering a critical window for early intervention.2–4 In fact, ED and CVD share the same risk factors including diabetes, hypertension, smoking and obesity.
