Letter to the Editor on "Urges to Drink Predict Urine Output Volume in Patients With Kidney Stones" Recently, we read with great interest the article by Liu et al. published in Journal, Urology. This cross-sectional study included 380 patients with a history of kidney stones and evaluated the correlation between self-reported cognitive evaluations of water, non-thirst-related urges to drink, and urine output volume. The results showed that the desire to drink flavorless water was positively correlated with daily urine volume, while the urge to drink flavored beverages was not related to daily urine output volume.

Reply to Editorial Comment on "Quality of Health Information Presented in Direct-to-Consumer Telepharmacies for the Treatment of Patients with Erectile Dysfunction" We thank Drs. Bal and Patel for their insightful editorial comment and for highlighting concerns regarding the adequacy of patient counseling and monitoring afforded by direct-to-consumer (DTC) telehealth care. The present study identifies a wide variation in the reliability of patient information on erectile dysfunction (ED) among the leading providers of DTC providers, underscoring the need for further investigation into the safety and efficacy of this nascent care model.

Management Dilemma for Very High-risk Non–muscle-invasive Bladder Cancer: Real-World Data Challenge the Guideline Recommendation for Upfront Radical Cystectomy We discuss real-world evidence that challenges the guideline recommendation of upfront radical cystectomy for all patients with very high-risk non–muscle-invasive bladder cancer. The evidence highlights the need for more accurate and dynamic tools to guide clinical decision-making. Until these are available, treatment decisions should be individualized and made in close consultation with the patient.

Intensive Intravesical BCG-induction in Patients with Asymptomatic Bacteriuria Asymptomatic bacteriuria (ABU) is common in adults with bladder cancer and does not require routine antibacterial therapy.1 Intravesical BCG is standard of care for high-risk non-muscle-invasive bladder cancer. We showed that BCG therapy is safe to administer in patients with ABU,2 and early responses were similar, and in some cases superior, to those in uninfected patients.3,4 This study evaluates the safety and efficacy of BCG in patients with ABU enrolled in a prospective study to receive sequential double-induction intravesical BCG treatments.
